Passport Health will be able to bill your insurance, but you must provide a photo copy of your insurance card and driver’s license or State issued ID.   Accepted insurance: Aetna, Blue Cross Blue Shield, Cigna, Humana and United Health Care PPO. If you do not have accepted insurance, the cost is $30 for the Quadrivalent Flu Shot.
